Policies

Full tuition must be paid in advance before the first lesson/class starts.
There is a one-time registration fee of $30 applicable to each family.
Tuition is Non-Refundable and Non-Transferable. Credit cannot be transferred between Academic and Music/Art/Dance Departments.
$40 shall be charged for each returned (bounced) check.
Credit is only valid for 12-months from the date of the last lesson. Any unused portion will be discarded after 12 months.
There are no make-up sessions for group classes.
All private, one-on-one lessons are subject to the "24-hour" cancellation rule. Meaning, any lesson cancelled within 24 hours of a scheduled lesson shall be charged, regardless. (Our instructors are committed to education and developing the individual; therefore, we respect their time, efforts, and expertise.
Virtual classes be recorded so that any students who miss a class can catch up on the missed assignments and be prepared for next week. Teachers & staff will actively monitor each virtual class for behavior & attendance, and to ensure the digital privacy and safety of your children during each session.
All private tutoring must be held at LIC/LISMA at 1125 Willis Avenue unless specified by LIC/LISMA.
You will be responsible for the penalty set by LIC/LISMA if the class location is other than 1125 Willis Avenue.

Liability Release

I assume financial responsibility for payment of classes and private lessons taken by my child. In case of emergency, and none of the contacts on this form can be reached, I grant LIC/LISMA permission to obtain medical attention for my child if necessary, for which I will be financially responsible.

COVID Waiver

I acknowledge the contagious nature of the Coronavirus/COVID-19 and that the CDC and many other public health authorities still recommend practicing social distancing. I further acknowledge that Long Island Conservatory/LISMA cannot guarantee that I nor my children will not become infected with the Coronavirus/Covid-19. I understand that the risk of becoming exposed to and/or infected by the Coronavirus/COVID-19 may result from the actions, omissions, or negligence of myself and others, including, but not limited to, LISMA staff, and other LISMA clients and their families.

I voluntarily seek services provided by Long Island Conservatory/LISMA and acknowledge that I am increasing my risk to exposure to the Coronavirus/COVID-19. I acknowledge that I must comply with all set procedures to reduce the spread while at Long Island Conservatory/LISMA (i.e. Do not attend class or tutoring session if I have any symptoms of illnesses such as fever, cough, shortness of breath, etc.)

I hereby release and agree to hold Long Island Conservatory/LISMA harmless from, and waive on behalf of myself, my children, and any personal representatives any and all causes of action, claims, demands, damages, costs, expenses and compensation for damage or loss to myself and/or property that may be caused by any act, or failure to act of the school, or that may otherwise arise in any way in connection with any services received from Long Island Conservatory/LISMA. I understand that this release discharges Long Island Conservatory/LISMA from any liability or claim that I, my children, or any personal representatives may have against the school with respect to any bodily injury, illness, death, medical treatment, or property damage that may arise from, or in connection to, any services received from Long Island Conservatory/LISMA. This liability waiver and release extends to the learning center together with all owners, partners, employees, staff and faculty.
